We get plenty of first orders but few people come back. What actually works for a small restaurant?

Start with making it easy to return: customers who order on your own site or app have an account, so you can reward them. DineHarbor includes loyalty points, stamp cards ('ten coffees, the eleventh is free') and a wallet, and it can collect reviews.
Keep the offer simple enough to explain in one sentence at the till, and show the progress on the receipt. Ask happy customers to review you. Then look at your reports after a month to see how many customers come back and what they order.
